Rural Care Coordination
ABOUT THE PROGRAM
Rural Care Coordination brings equal access to residents by providing innovative care coordination to children and youth who are experiencing emotional and behavioral difficulties in the home, school, and community settings. Barriers in accessing mental health services, including affordability and long wait lists, are magnified for families living in rural areas due to geographic isolation and lack of transportation. A client Coordinator provides individualized care that reduces these barriers, making mental health care more accessible. Rural Care Coordination staff provides a continuum of mental health care to children, youth, and families by identifying strengths, needs, and concerns. Families, community providers, and school staff all work together to develop an action plan for success.
Rural Care Coordination serves the following school districts: Windsor, Harpursville, Whitney Point, Deposit, Chenango Valley, Chenango Forks, Susquehanna Valley, and Maine-Endwell.
Children, youth, and families welcome the opportunity to take ownership in their circumstances and work toward the following goals to enrich their quality of life:
- Attend mental health appointments
- Reduce the need for crisis intervention services
- Improve academic performance and increase school attendance
- Access appropriate community resources to improve circumstances
